• Cellular Arrhythmogenic Effects of Congenital and Acquired Long-QT Syndrome in the Heterogeneous Myocardium -- Viswanathan and Rudy 101 (10): 1192 -- Circulation: We investigated the effects of reducing IKs or IKr or enhancing late INa (to simulate the 3 forms of long-QT syndrome) on action potential duration (APD) in the context of IKs heterogeneity. We introduced IKs heterogeneity in the Luo-Rudy dynamic cell model to simulate epicardial, endocardial, and midmyocardial (M) cells. Results demonstrated higher susceptibility of M cells to the development of arrhythmogenic early afterdepolariz ations (EADs) in isolated cells and poorly coupled tissue. An important observation is that IKr block or late INa acts to increase APD differences between the cell types, whereas IKs block minimizes such differences. Also, for normal transverse coupling, EADs develop in the endocardial region rather than in the M region as the result of strong electrotonic interaction. Conclusions?IK s heterogeneity and intercellular coupling strongly influence EAD development during interventions or disorders that prolong APD. M cells in isolation or in poorly coupled tissue

• Sodium Channel Block With Mexiletine Is Effective in Reducing Dispersion of Repolarization and Preventing Torsade de Pointes in LQT2 and LQT3 Models of the Long-QT Syndrome -- Shimizu and Antzelevitch 96 (6): 2038 -- Circulation: Methods and Results A transmural ECG and transmembrane action potentials from epicardial, M, and endocardial or Purkinje cells were simultaneously recorded in an arterially perfused wedge of canine left ventricle. Higher concentrations of mexiletine (10 to 20 µmol/L) totally suppressed stimulation-in duced TdP. Conclusions Our results suggest that although pacing and sodium channel block are very effective in abbreviating the QT interval and TDR in LQT3, these therapeutic approaches may also be valuable in reducing the incidence of arrhythmogenes is in LQT2.

• Cellular Basis for the ECG Features of the LQT1 Form of the Long-QT Syndrome : Effects of ß-Adrenergic Agonists and Antagonists and Sodium Channel Blockers on Transmural Dispersion of Repolarization and Torsade de Pointes -- Shimizu and Antzelevitch 98 (2: Conclusions?Th is experimental model of LQT1 indicates that a deficiency of IKs alone does not induce TdP but that the addition of ß-adrenergic influence predisposes the myocardium to the development of TdP by increasing transmural dispersion of repolarization , most likely as a result of a large augmentation of residual IKs in epicardial and endocardial cells but not in M cells, in which IKs is intrinsically weak. Our data provide a mechanistic understanding of the cellular basis for the therapeutic actions of ß-adrenergic blockers in LQT1 and suggest that sodium channel block with class IB antiarrhythmic agents may be effective in suppressing TdP in LQT1, as they are in LQT2 and LQT3, as well as in acquired (drug-induced) forms of the long-QT syndrome.
• Effects of a K+ Channel Opener to Reduce Transmural Dispersion of Repolarization and Prevent Torsade de Pointes in LQT1, LQT2, and LQT3 Models of the Long-QT Syndrome -- Shimizu and Antzelevitch 102 (6): 706 -- Circulation: This study examines the effects of nicorandil, a K+ channel opener, on transmural dispersion of repolarization (TDR) and induction of torsade de pointes (TdP) under conditions mimicking the LQT1, LQT2, and LQT3 forms of the congenital long-QT syndrome (LQTS). Conclusions?Ou r data suggest that K+ channel openers may be capable of abbreviating the long QT interval, reducing TDR, and preventing spontaneous and stimulation-in duced TdP when congenital or acquired LQTS is secondary to reduced IKr or IKs but less so when it is due to augmented late INa.
